Vaccine Information
Polk County Health Center urges those who are 5 and older to vaccinate against COVID-19. PCHC provides Moderna vaccine for individuals 18 and older and Pfizer vaccine for those 5 and older. Pfizer and Moderna 3rd dose and booster dose is available for those who qualify. Individuals who are immunocompromised, pregnant or breastfeeding should consult with their doctor before getting any vaccine.
Vaccine is available at Polk County Health Center Monday-Friday 8-11:30am and 1-4pm. PCHC is also available to schedule onsite vaccine clinics throughout the week.
