How Do I Know If I Qualify?

1. I am a woman.
2. My income is at or below 250% of the federal poverty level for household income (net – after taxes)

$39,125 for an individual

$52,875 for a family of 2

$66,625 for a family of 3

$80,375 for a family of 4

$94,125 for a family of 5

$107,875 for a family of 6

$121,625 for a family of 7

$135,375 for a family of 8

AND

3. Age 21 to 64, or older if you do not receive Medicare Part B, AND
4. No insurance to cover program services.

 If a test comes back abnormal, these diagnostic services are provided at no cost:

Diagnostic mammogram

Ultrasound

 Breast biopsy

 Fine needle aspiration (a type of biopsy)

Conization (a surgery that removes the abnormal, precancerous cells from the cervix

Transportation vouchers to help with cost of getting to appointments
